Evaluation of Dental Implants Exposure Techniques at Second-Stage surgery.

Abstract

Background:Morphology of the peri-implant soft tissue adjoining the implant components plays a pivotal role in displaying the implant esthetics. Creating an implant restoration that cannot be distinguished from the rest of the natural dentition is the ultimate goal. Second stage surgery is often overlooked and is considered non-essential phase but actually could determine the health of the peri-implant tissue .This phase gives an excellent opportunity to preserve, reconstruct and even maneuver the soft tissue to optimize the soft tissue profile around the implant components. The article aims at enumerating the various modalities available to contour the soft tissue profile around implants and thus help the practioners in optimizing esthetics during second stage surgery. Purpose: This study   was intended to   evaluate  implant  exposure  techniques  (incisional  and  excisional) at  second-stage surgery. Patients and Methods: Twenty  four  patients  in  need  for  surgical   exposure of  second-stage  implants   was chosen  for  this  study  from  available  cases in Oral  surgery  dept., Faculty of dentistry, Mansoura  university, Patients was be classified  into  two groups: Group 1:Twelve patients in whom incisional techniques for  implant exposure  had been done, Group 2: Twelve patients in whom excisional techniques for   implant exposure  had been done. Patients  was  followed  clinically   for  healing immediately   and   after  surgery   and   3 months  after  surgery. Due to concerning healing such as cosmotic effect in anterior region, rate of healing, inflammation, bleeding, pain  and  other complications will   be recorded   and evaluated   statistically. Results: Statistical analysis showed significant difference between the two groups. Exposure by laser is the best technique followed by punch technique. Cross-shaped technique is the least technique according to escthic appearance and pain. Conclusion: There are various innovative methods for promoting and preserving the soft tissue profile around the implants. Second stage surgery should be given emphasis and not just thought as a process of uncovering the covers crew. A whole lot can be done and is an excellent opportunity to give finesse to the soft tissue profiling around implant components. The biological, functional, esthetic needs of the individual patient can be sculptedwith this essential step. Keywords: Dental implants, esthetics, second stage surgery.
