Stress Transmitted To Palatless Overdenture Retained By Four Implants Splinted With and Without Cantilever Bar (Strain Gauge Evaluation) (In-Vitro Study)

Abstract

Aim of the study: the aim of this study was to compare between the stress that transfere to peri implant area by bar versus bar with cantilever. Material and Method: four implants was placed bilaterally in canine and second premolar regions of edentulous acrylic resin maxillary cast and connected by bar with  7mm posterior cantilever extension and soft liner was used as a female housing for the bar, eight strain gauges were bonded to the acrylic resin at the mesial and distal surface of each implant. Stress was measured with cantilever extension by using straingauges, then the 7 mm cantilever extension was trimmed and the stress measured for the bar only. Results: Bar with cantilever extension demonstrate a significant increase in stress compared to the bar without cantilevres in unilateral (UL) and bilateral loading (CL). During unilateral loading in bar with cantilever extension and bar without cantilever the maximum stress was recorded at distal side of posterior implant of loading side. The result show that bar with cantilever extension demonstrate a significant increase in stress value at peri-implant area as indicated by Mann Whitney test (p≤0.05) in loading side, the higher stress value was recorded by distal side of posterior implant (PDI) with means value 414, but there is in-significant difference for non-loding side between both groups except at mesial side of anterior implant (AMI) which is p=0.001. Conclusion:the four implant connected with the bar without cantilever extension demonstrate the lower stress than with cantilever extension.
