Study of Corneal Endothelial changes following Divide-and-conquer versus stop-and-chop Phacoemulsification techniques

Abstract

Purpose: To evaluate and compare changes in corneal endothelial cells after divide and conquer versus stop-and-chop phacoemulsification techniques.
Methods: The study was conducted on 90eyes with senile cataract that were assigned to have phacoemulsification and posterior chamber IOL at Mansoura ophthalmology center in the period from May2019 till June2020. The eyes were divided in to two groups; each group had 45eyes. One group underwent phacoemulsification with the stop-and-chop technique. The other group underwent phacoemulsification with the divide-and-conquer technique. All individuals were subjected to full history and ocular examination. IN addition specular microscopy was performed preoperatively, one week, one month and three months post-operatively to assess endothelial cell counting.
Results: There was statistically significant decrease in the mean ECC in the two groups post-operatively at 3month(P <0.05*) compared to pre-operative mean ECC. In addition, There was statistically significant decrease in the mean ECC in the Divide and conquer group post-operatively at 1week(P <0.014*) and 1months(P <0.012*), compared to pre-operative mean ECC. However, difference between the two groups was statistically insignificant(P=0.163 after one week, P=0.133 after one month and P=0.256 after three months).
Conclusion: less endothelial cell loss occurs after 1 month in both groups and least decrease was after 3 months (about 9.9% in divide and conquer group and 8.7% in stop and chop group as compared to pre-operative mean ECC). Although less endothelial loss occur in stop and chop group (about 8.7%) when compared with divide and conquer group (about 9.9%) the difference was statistically insignificant.
