Expression of Lymphocyte Homing Associated Cell Adhesion Molecule [H-CAM, CD44] in B-Lineage Neoplasms

Abstract

Adhesion molecules [AM], such as C44 and its variant isoforms, are known to play a pivotal role in cell-cell and dell-stroma interactions involved in proliferation, spread via blood and lymph vessels, tumor invasion and metastasis. This work was aiming to study the a expression of CD44 adhesion molecule in different B lineage-cell neoplasms in order to understand and predict the biologic behavior and heterogeneity of these disorders. The study included thirty patients with:B lineage neoplasms, eleven with acute lymphoblastic leukemia, nine with chronic lymphocytic leukemia, one patient with hairy cell leukemia and nine with lymphoma. They were 21 males and 9 females. Their ages ranged between 3-73 years. Ten healthy individuals are included as control group. Immunophenotyping on peripheral blood [PB] or bone marrow [BM] samples using a panel of monoclonal antibodies to select patients with clonal B lymphocytes neoplasms as well as expression of CD44, using either PB or BM mononuclear cells, were performed by flow cytometry. A significant relation between CD44% and its mean fluorescent intensity [MFI] was observed in ALL, CLL versus the control group, negative correlation was found between blast % in PB as well as TLC and CD44% in ALL group. Negative correlation between CD44% and platelet count and positive correlation between MFI and lymphnode enlargement were observed in lymphoma group. Significant difference between CD44% in low grade and high grade lymphomas versus CLL was noticed. Also a significant difference was found in MFI on comparing low grade and high grade lymphomas versus CLL and highly significant results were found between intermediate grade and CLL. In summary, the data presented in this study may help to clarify the qualitative abnormalities of CD44 expression by neoplastic B-lymphocyte and confirm that specific patterns may be of value to refine the diagnosis of B-cell lymphoproliferative disorders.
