Abstract

Egypt is experiencing significant age structure changes that will have major implications for its socioeconomic development. The most significant changes are among the older population, which quadrupled over the period (1950-2010) and is expected to quadruple again over the period (2010-2050). The current study was conducted in the outpatients' clinics of the Manial University Hospital (Kasr el Aini outpatients ' clinics ); Cairo University .It aimed to identify the types of their morbidities, some factors underlying occurrence of these morbidities, assess their unmet health care needs through the INTERMED for the Elderly (IM-E)questionnaire. Finally, to design a plan of outpatients' services to old people to fulfill their unmet health care needs. This study is a cross sectional descriptive study. An in-depth qualitative part was also performed through Focus Group Discussions "FGDs" and in-depth interviews with subsamples of the studied elderly and health care providers .Five hundred elderly persons above sixty years old were selected randomly at the registration desks . and the following data were collected: 1-Personal and socioeconomic data.2- Risk factors in the life style of the studied elderly participants.3 The chronic diseases trends among them.4-Their functional ability.5-Health services accessibility and their satisfaction.6-some clinical indicators like: blood pressure measurement, random blood glucose and BMI.7- Finally, assessment of health care needs of elderly using the (IM-E)for elderly questionnaire. The study revealed that majority of elderly (81.6%) had clear diagnosis and (57.2%) had chronic diseases. Blood pressure measurement revealed that (37.8%) had uncontrolled blood pressure measurements (≥ 140/90) and (23.2%) of elderly had uncontrolled blood glucose levels. The majority of the studied patients were either overweight (33%) or obese (53.6%). These diseases caused either partial (48%) or complete (26.4%) inability to perform function.Half of elderly (50%) are expected to suffer from mild psychiatric disorder in the near future. Half of elderly had no change in living situation (49.8%), but additional home care by sons, daughters or social facilities were needed. Majority of elderly (79.6%) believed that additional efforts to organize health care are needed. Establishment of a new geriatric health clinic with multidisciplinary clinical team for diagnosis ; treatment ; follow up of elderly cases was recommended.
