Abstract

Background: The daytime fasting during the lunar month of Ramadan is observed annually by millions of adult Muslims all over the world.Objectives: So we aimed in our study to assess the effect of fasting on fluctuation of blood pressure in primary hypertensive patients.Methodology: We selected thirty well-controlled hypertensive subjects (9 women and 21 men) aged 33 to 69 years on regular once or twice-daily anti-hypertensive medication who had controlled blood pressure <140/90mmHg. None of these patients had other severe disease to be exempt from fasting. Each subject was instructed to fast during the month of Ramadan while wearing the ambulatory blood pressure monitoring device on his or her non dominant arm for 24 hours. Before fasting, 24-hour ambulatory blood pressure monitoring was performed for each of the patients in another non fasting day. During the fasting state, they advised to take their usual antihypertensive medications early in the morning (before suhoor) and/or after sunset (according to their previous timing schedule as once or twice daily).Results: We have found statistically significant decrease in blood pressure in hypertensive controlled patients during fasting in comparison to the same patients in non fasting day.Conclusion: This study revealed that treated hypertensive patients on once or twice daily medications if well controlled, possibly by having home recording or frequent office recordings, can be assured that with the continuation of their previous medication(s) they can also perform traditional fasting without any harmful rise in blood pressure variables.
