Effect of single implant versus two implants mandibular retained overdenture on patient satisfaction and biting force of completely edentulous patients : A randomized clinical trial

Abstract

Statement of the Problem:The cost of rehabilitation of the problematic mandibular complete denture with an implant-retained overdenture, or implant-supported fixed prosthesis, is outside the financial scope of many compromised denture patients especially in developing countries. A more affordable treatment alternative is therefore desirable. Purpose: The purpose of this study was to ascertain whethersimplifying mandibular overdenture treatment utilizing two stages surgery and delayed prosthetic loading of a single implant, willachieve similar patent satisfaction rates and maximum biting force similar to that expected using conventional technique with mandibular overdenture supported by two implants.Materials and Methods:A 28 completely edentulous patients were selected with mean age of 58.2 years. For all patients Complete denture was constructed in usual manner and they are assigned randomly into two equal groups. Group1 received one midline implant whereas group 2 received two interforaminal implants to retain the mandibular overdenture. Patient satisfaction was assessed using100mm VAS scale andbiting force was assessed by using load star sensor, for all patients at baseline,3,6 and 12 months follow up period.Results:after one year the result of patient satisfaction reveal nonsignificant difference between two groups regarding general satisfaction, esthetic and speech items throughout the following periods baseline,3,6,12 months (p0.05). Regarding the stability and chewing ability the two implant group show increase values in comparison with single implant group at 3,6,12 months follow up periods(p 0.05).The biting force of G2 shows great values than group G1 at 3,6,12 months follow up periods and this difference was statistically significance (p 0.05).Conclusions: Within the limitations of this study, it appears that the single implantretained mandibular overdenture, can provide a beneficialtreatment outcome over a one year observation period.
